Haven Pelvic is out of network with commercial insurance plans so your therapist can provide a full hour of individualized care without balancing multiple patients at once.

Medicare Patients

Haven is a non-participating Medicare provider.

This policy began January 1, 2026. Please call our office before scheduling so we can review the correct Medicare-approved pricing with you.

  • You pay Haven Pelvic at the time of service.
  • Our office submits your claim to Medicare on your behalf.
  • Medicare reimburses you directly for the covered portion, subject to its allowed amount, deductible, and coinsurance.

Have Medigap coverage? Your supplemental plan may reimburse some or all of the remaining eligible amount after Medicare processes your claim. Contact your supplemental insurer to confirm that your benefits are coordinated. For Medicare pricing, call 908-228-5561.

Whether you plan to use out-of-network benefits or pay privately, our team will make the process as straightforward as possible.

Using Out-of-Network Benefits

Submit your superbill for possible reimbursement.

Payment is due to Haven at the time of service. Your superbill will be available through the patient portal so you can submit it to your insurance company for out-of-network reimbursement.

Before your first visit: Call the member services number on your insurance card and ask about your out-of-network physical therapy benefits, deductible, and reimbursement process.

Self-Pay

You can receive care without out-of-network benefits.

Many patients choose Haven because they value specialized pelvic health care, even when their insurance plan does not include out-of-network coverage or has a high deductible.

Receipts are available for eligible HSA or FSA reimbursement. Packages of 10 visits are also available; contact our office for current package details.

Your insurance coverage does not determine whether you can become a Haven patient.
