When registering, enter the exact spelling of your first name, last name, and email address that Haven Pelvic has on file.
If the information does not match, the system may create a duplicate patient profile. If you are unsure what information we have on file, please contact us before registering.

Choose this option if you are an established Haven Pelvic patient and have been seen within the past 12 months. You will be charged $200 to reserve your slot.

Choose this option if you have never been a patient at Haven Pelvic or if your last visit was more than 12 months ago. You will be charged $250 to reserve your slot.
